Super Mutant - Lycanthropy of Fallout.
Biggest hurdle is probably re-writing all the dialog of the game to take into account the changes. For example, you wouldn't really be welcome in Diamond City (Which in theory would break the main quest)
But yeah, would be cool.
I totally agree and have thought of the Diamond City dilemna, two thoughts come to mind.
1. Can't go there: Simple as that, a few story quests would have to be altered.
2. Persuasion: Persuade the guards to let you in, then convince Mayor McDonough, I mean they let Hancock in right?
Attatching a mutated version of ghoulism to radiation dmg or using built up radiation to transform into slightly larger, greener, tank would be cool.
I also hope one of the future DLCs lets us put our mind in a Synth body.
Very interesting idea, being a synth would be very fun!
Being a Super Mutant is a bit more complicated. One would expect you to be attacked everywhere but GoodNeighboor and I guess any of your settlements. I guess with Virgil's quest, it's possible to reverse it, so really you're just looking at a niche RP thing.
If I had to choose one or the other, I'd rather have Ghoulification.

Ghouls and Super Mutants aren't races, they're mutations. I don't think the same standards should apply in this case. (One isn't born either)
I know that, but the same same idea. Instead of being a human in a Vault, your character creation starts with you in the wasteland choosing if you're human, ghoul or watever.
It's interesting, but that brings a lot of angles to the story and the world that might make it difficult.
Still, I remember in Fallout 1 and 2 that too much rads would give you mutations. I grew another toe. That's something I'd wish they bring back as a far as mutation goes.
But oh, the poor voice actor's vocal chords...
On the plus side the dialogue wheel wouldn't be an issue anymore either, as they are not the smartest lot, on the negative side it'd mean calling back in old voice actors to repeat lines for the mutant version of you, should you stay mutant. (For any DLC, not mods obviously)
Also on the plus side you could make some super sized weapons for mutants only, and some kick butt giant powerarmor, dual miniguns anyone?
Could you put some sort of audio filter to make a voice more ghoulish?
At that point you'd have to cure all super mutants and infect all humans, just for the joke.
I do miss my extra toes...who knows maybe you can regain your mutated limbs in a future mod
