I would also recommend against directly fast-traveling to the Fallon's Department Store / Milton General Hospital POI cluster. Seen up to three super-mutant suiciders there...
Those two are fine if you are stealthed, its the West Roxbury station in that cluster is almost instant death. I thought I had cleared it recently enough popped in there today, I made it out with like 20 hp after two suiciders hit nearby.
Yes, three suiciders is the standard spawn.
Least the land mines dont respawn like the suiciders, lol
I believe it is intentional.
If you use console commands to moveto the random NPCs like Kelly and Gene it places you in a small room wiht ALL of them. Well by default all the random NPCs are killable, except Ness who is hostile to all of them and has the essential flag. So basically she will kill them off 1 by 1 if you go in there with console commands.
There was a post last week about a guy who got trapped in Medford Hospital because the outside mobs respawned while he was inside. So he ended up exiting next to a suicider.
Why not both?

Problem is easily solved by just fast traveling to the building next door. It's like three seconds difference.
I killed the suiciders outside the hospital and one of the suiciders inside the hospital, but there were just too many mutants in there for where I'm at right now, so I left it alone.
I play stealthy, so if I see a suicider before they see me, I just tell my companion to run out in front of them at a distance, so the suicider explodes on them and the blast is wasted. Backfired on me once, but it's worked on a couple of the suiciders so far.
