ps4 pip boy glitch PLEASE HELP

When i open my pip boy,the screen of the pip boy goes all weird,and it looks like somebody took the screen off,melted it then put it back on. Everything in the pip boy is distorted and prevents me from reading anything so it makes it really hard to do anything , even swap weapons or fast travel

It turns out that my disc was dirty,its fine now
