Just because your other mods work doesn't mean that adding another *massively intrusive* mod like a start mod won't break literally everything.
If it's enabled there, and not working in-game, then you should ditch SMU. SMU hasn't had an update in 5 years (2018) anyway so you should instead get the "Redux" version, which is still being updated.
If even the "Redux" version doesn't work, you have major mod conflicts or haven't set your F4SE and/or other mods up properly.
You are launching via F4SE? and you've made the custom.ini edits noted below, to enable the game to use mods properly?:

Thanks for this. Just getting back into FO4 (haven't played yet) and just setting up some baseline mods. I knew I had to do this but couldn't remember exact step. TLDR. Anyway, thank you.
