It has to be with you working with ALL of them somewhat equally, and long before any end-game things happen.
I'd suggest perhaps, put it in a different part of your load order and make sure that there are NO other overarching mods that may have affected any outcomes. Find a manual save from well before you might have chosen to work only with one group, and try again from there.
It's a very finicky mod, indeed. But it's best to have it installed and working properly before you start getting too close to the turning point. If piper thought something had happened, also you might check your completed quests list in your pipboy. I can't remember what might trigger that specifically by name.
Q: How does Paladin Danse fit into this mod?
A: As in vanilla, he stops being your companion if you become hostile with the Brotherhood of Steel. If you replace Maxson and become Sentinel, he can become your companion again, albeit under very different rules. He will still hate you but affinity will no longer matter. He can be taken alongside other companions as he is no longer using the old companion system. If he was exposed as a synth before Maxson was replaced, Synth Maxson will reinstate him. If he becomes Elder (Danse Dilemma mod outcome) you can have Father and SRB reset him to avoid having to destroy the Prydwen. A reset Danse may become your companion if you convince the SRB to allow it. Paladin Danse also rejoins you if Sarah Lyons reinstates you into the Brotherhood.

anyway, as you have to follow the institute path to take it over subsequently you have to betray the BoS at Mass Fusion... Danse sees this as if you would betray him personally and will hate you for that.
Maxon may reinstate you and give you the Sentinel Title that allows you to Boss Danse around... but he will still think of you as the Traitor.
Same if you become Elder via the Danse dilemma Mod.
However, if Danse him self becomes Elder and you reset him by talking to Alana Secord he will forget about that and gets the impression you were just undercover of sorts.
No hard feelings and he will even be very cordial when adressing you (almost like a romanced character).

I know, the problem for me personally is that I have never been able to stand dealing with Maxson long enough to resist shooting him in the face.
you know... like degrade him into your personal Cup bearer...
but thats JUST BUSINESS.
As Danse is not a Vanilla Companion anymore he has to go back to his Spawn Point.
Same for Glory, Mutant-Virgil and Z-Something you work with when doing the Railroad Escape...
As hes is the Elder... just pretend he has important BoS Work to attend to.
