Just mods. There are literally thousands of Settlement mods out there, I use hundreds of them.
Some games HAVE base building, such as No Man's Sky and Subnautica, but then again Fortnight had building in it i guess until recently I don't know I don't play battle royale games.
Tower Unite, Garry's Mod, any given city simulator... lots of options in various themes.
Like Valheim and No Man's Sky have some great building features in it, but the bases feel incomplete because you have no settlers wandering around your creation.
Which I think this is the answer you are looking for:
Sim Settlements and Horizon are two overhaul mods that change the experience of the game but still has that same basic base building.
Maybe try Conan Age of Sorcery then... its dark fantasy survival rather than sci-fi RPG, but has pretty good building tools and you can populate your builds with people and creatures that you capture/tame.

The catch with big is that the AI in the game isn't smart or powerful enough to get all those settlers living realistic lives.
Sim Settlements 1 or 2 are other options.

If someone could do a mod where the junk you are carrying gets shown in-game that would be a mod I would chip in for.
Only thing is, if you're looking for a game like Fallout 4, MD has very little combat in it and content is lacking, though the devs are working on it all the time. Also (I would think "obviously" given its name, but...) it's set in a fantasy medieval world rather than a PA wasteland. I think it should still be in beta, or early access at least, but it is playable and engaging. I believe the game uses the same engine KCD uses. The graphics look very similar anyway.
I like the game pretty much, and I never thought I would. I'm not a huge building fan, but I like it when there's a nice, violent game surrounding it. But other than fighting off the random anemic bandit, or grizzly bear, you'll be spending most of your time building your village and keeping your villagers happy.
If you want to build up your base/town/city and watch your people doing things,. there are a number of games that do that. Both modern settings and old. "The Settlers" being another one to add to pile mentioned so far by the others
But if your looking for a game that "plays like FO4" with a single player character with base building, your probably better off looking at mods for this game than trying to find another one.
There are ALOT of base building mods, so, id experiment with them...
