This never happened before but for some reason my mouse sensitivity is extremely slow when looking up and down. Editing Fallout4.ini has no effect in game... Help?

Which ini are you editing, as there are several.

What is your game's resolution and does it match your native screen resolution? What's the FPS you're trying to get?

If you played before, and now it's not working, what changed on your computer? Did you change hardware, update drivers, an OS update, etc?

I'm trying to get the sensitivity to be the same in x and y axis. I'm editing Fallout4.ini. My resolution is 1920x1080.

I have done nothing with my PC recently. Only change is new GPU drivers since they update automatically

Then perhaps revert your driver, because if that's the only thing that's different... it is likely to blame.

math is simple

your res: 1920x1080
fallout4.ini >
fMouseHeadingXScale=.021
> 1920 * 0.21 / 1080 = .03733

fMouseHeadingYScale=.03733

Like i said, it makes no difference. I can change it to 1000 and it makes no difference

try setting the Fallout4.ini, Fallout4Prefs.ini and the custom one, if it exists, to read only.

(must be some mod or the mod manager which is interfering with it, had the same issue with fov value. it would remain at the right value at all times, but it was ignored)

I figured out the problem. I had used the saveini command which had created an ini file which prevented any of the other ones from reading. Similar thing happened with Skyrim but it was a bit more obvious in that case. Deleted that file and things now seem to work as intended

Awesome, and thanks for sharing the fix. While it might not happen to every user, it's certainly one of those things that you'd have to really scratch your head figuring out.

Commenting so I can find this later, kinda found the y axis being different annoying.

That's been the norm for a generation, honestly. Weird, but still been that way as long as this has existed. Your screen's not square.

Maybe if you started the damn truck your mouse might fix itself

You can find them in Documents\My Games\Fallout4\

However, best might be to use Bilago's Configuration Tool. It is a stand-alone app that works in the same way as the original launcher but is much better. It lets you modify all known settings, has a graphical user interface for the common ones, can make .ini files read-only, can replace itself with the original launcher, lets you choose a personalised background picture and more. You can change resolution, display mode, mouse settings, graphics settings, HUD colour, etc.

I have tried everything i could find on the internet to try and solve my problems, nothing is working. Also tried using the config tool but nothing. Y value always slower than X. My Fallout4Custom ini looks like this:
[Controls]
bMouseAcceleration=0
fMouseHeadingXScale=0.021
fMouseHeadingYScale=.03738
fMouseHeadingSensitivity=0.03
fIronSightsFOVRotateMult=0.75

Has also been set to read only mode.
