Many times I have had SMs attack Vault 88.
I just open them all because the multiple exits and entrances are useful, particularly in Survival. I move the defence focus to the central area rather than the perimeter. It's never a big deal.
5 blocks can completely cover an entrance. Snap 5 of them each other in an open area and group-select them and move them to a tunnel entrance. no holes will be left around them.
And yes enemies can spawn inside vault 88. These are the locations near the main atrium tunnel that connect to main vault 88 entrance.

Didn't even stick any turrets outside, they were just in the lobby by the door. No turrets in the caves either, but i blocked up doorways with concrete walls anyway. No access from caves into the vault though, so that might make a difference....
I do leave the Vault door open and about 6 turrets and one guard with a mini gun take care of the attackers fairly quickly. None have ever made it pass the Vault's entrance. My char just fast travels there and watches the attack for a short bit.
They will go by one or both of those areas eventually. No need to worry about them getting too far if there's a line of turrets at both points.
Defense score determines the likely hood of an attack to happen
The higher the score the less likely it will happen.
If you have a Settlement is under attack notice if I remember right you got 3 days to go to the settlement.
if you don't arrive in 3 days its considered a loss and it will have some sort of damages
If you arrive Early - the enemies spawns at specific locations
If you arrive later - enemy will already be inside

Like all settlement attacks, they spawn at specific points regardless of your doors or walls. Which is why sometimes they might get stuck in things, behind or under platforms, etc. They don't walk to the place they literally just teleport when the game says 'now attack'.
That said, if you're waiting for them I find it best to leave a single path open, because then when the game does bother working out their exact movement it favours that path instead of hitting a block everywhere and eventually teleporting them as a last resort.
