If your gpu is at 90% load then it is probably bottlenecking your pc. So the cpu doesn't need to run fast.
But if you have the fps capped and you are running at the cap, then the 90% just means that the gpu is at close to maximum performance. The cap is the bottleneck.
If the cpu was not bottlenecked by the gpu, fps cap, vsync etc then as FO4 has only 3 main threads of code, then the % load is unlikely to be high in a modern pc which has many cpu threads. So a % cpu load doesn't mean that much.
If you waited 7 years to play this and it still runs like ♥♥♥♥, did you also wait 7+ years to upgrade your pc? Even my old pc runs this at 60 fps on max settings.. lol
