if you have a high refresh rate monitor, vsync might just be capping your fps to THAT framerate, instead of the locked 60 the game actually requires to work.
i just cap my in-game fps to 60 by other means. i dont actually think you can change the refresh rate of fallout 4, and changing your actual monitor refresh rate every time you wanna go on another game is a pain. are you on nvidia or AMD?

cant help you personally since i'm on AMD.
EDIT: also make sure to actually MONITOR your framerate using shadowplay or something. its important to know if its working or not
