I haven't played FO4 for months now, btw (keep meaning to go back) but it's interesting to see this thread continue.

It's from one or more glitching ghouls that fell through the ground, or appeared on some other inacessible spots. Some circumvented that via console - what can cause follow-up bugs coming from that (but these can be circumvented or ignored again). If you are not playing on PC, or play on survival, there is of course no command-console. For that the only verified way I know of is via "the molecular level", but "only" if it's done exactly the right way, as depicted some posts back. Everything else I read on the web (Again: I don't speak about console commands) is often just hearsay or just seemed to work for someone... because it didn't happen to him at all that a ghoul glitched through the ground and the specific run. For some it does even never happen (as far as they are aware of). Until it happened. This way several "theories" have been broken, but it could quite fast be pinpoint to a (most often single) ghoul that just can't be reached or shot, making the quest unsolveable. A few folks have digged into that with the console, when it happened in a regular walkthrough, so they found the unreachable ghoul. And without console there is no other way than the molecular level.So if you already "did" that quest "before" even doing this very specific quest Crossroad_Demon14, I see no other way to make it work furtheron in this direction.
some of the missions seem to just randomly get bugged lol.
On my earlier playthrough it was the first concord mission,the game wanted me to kill raiders that didn't exist. There was just red quest markers floating in the air.
Ghouls falling through the ground are just rng, but if she downs the dialogue is bugged and you can't proceed, so restarting and keeping her alive is the way to go.
Level 30 is not really that far into the game, so that might be the best thing to do. XD
I can confirm this is a major cause for the bug as well. For those who cannot solve this issue with Jojo's method, IF Scribe Haylen is down, simply open the console and click on her, and type "resurrect". She will respawn and the quest will correct itself.
I would also like to mention that this should be done before trying Jojo's fix, or rather, make a different save for both methods. If Jojo's fix doesn't work, doing this method on the same save will only break the quest further.
