I looked it up on YT. They said weakness comes from going 5 days without eating or drinking. In my case that was not true because I had plenty of food and purified water.

and I'm pretty sure I got after a buff/medx/psych/jet rampage against some skullies and a legendary when I wasn't wearing PA.
I have C6 and Local LEader 2 just so I can setup med vendors and pay them 15 caps to "Cure ME" in my forward settlements. Have not used antibiotics since I hit lvl 14.
Yeah Vulture, I usually get that set up asap but, this is a new playthrough. I've actually found a good ammount of AntiBs this run so I'm holding off till I can build the tier 3 doctor. I built the the level 1 asap my last run. ;)
Some of these diseases and illnesses are probably still in play by the devs so, I guess I'll just have to wait and see how it all pans out.

Actually sleeping is when illnesses catch up to you. Things you did that day catch up to you when you sleep unless it is an immediate illness caused from damage taken. Even eating certain foods will catch up to you (like parasites and such.)
Think of it like an incubation period and the illness breaks out later.
Ahh that makes sense and is a pretty cool system if that's the case. =)
