Then there is Poseidon Energy, another huge and powerful corporation that we don't get to see much of. They have always existed on the periphery of the story and haven't taken a direct role at any point...so far.
Both of these groups had the resources, the know-how and had contingency plans.
Then you have space aliens. Since they are real in this game, who knows what kind of role they might have played? There could be another powerful faction waiting on another planet or living on space ships.
Lastly, there are all of the people who made it by hiding out in sewers, caves, basements, etc. We find them in every game.
There are lots of options, the writers were clever that way - anything is possible. Vault-Tec was running an experiment, experiments have controls, some of the vaults are working on a larger timeline and others may have no "end-date" whatsoever? We haven't seen the vault that had the world's most powerful and influential people in it, the one stockpiled with valuable resources, tools and knowlege needed to rebuild the world (200 years counts as nothing when talking about the survival of the human species).
We don't know and they can do whatever they like. They have laid the groundwork for as many more games as they can stand to make. As long as there are players willing to spend their money, there will be people willing to make us more games.

On the bright side, in F3 enemies couldn't open the door, so you could hide from them in one until they went away
Imagine being locked in one, needing a crap, but the doors blocked with debris.
