I'm pretty sure it's stock, haven't ever played with it, not that I can recall. Yeah I've gone from low/ultra to everything in between, no noticeable difference.
And does this issue persist when you are inside interior cells?
True enough, it might be a driver issue, though like I said I was playing just fine until the middle of the session and I just wound up getting the issue. Doesn't seem to appear when I'm in interiors, leading me to believe it's got something to do with the way the game's rendering shadows when outdoors. I'll try playing around with my drivers I guess, see if that fixes it.

I literally never found a solution, I dug through every dirty corner of the internet to try and find one and nobody could tell me anything. The game's a technical mess.
