Wish we could do more with Dogmeat, though. Hats, sunglasses (instead of just welding goggles), and doggie shirts, maybe. Some deely-bobbers, perhaps.
Equip option is available if trade dialogue option is available and you are able to look at the personal inventory of an NPC, but be aware that, if a settler is currently working as store keeper, their trade menu will only let you trade based on value, just like other traders. These settlers will only be available to switch equipment once they close their stores.
For any other case, simply talk to the NPC, and they will open their inventory for you to modify what they are currently equipping.

That's only true for clinics. Any other storekeeper will have both "trade" and "barter" dialogues, and the trade one is the usual equipment-swapping.
Clinics are an exception because they have two store-related options -- "barter" and "cure me" dialogues, so you do need to catch 'em when they're not working.
I've never had a problem with changing clothes on a trader who was still working at his store. If a script is running, yeah, you have to wait for it to finish to be able to do anything with that NPC, but otherwise, I have changed their clothes and equipped them with better weapons while they were at work.
And yeah, doctors are a special case.
They're treated like any other settler, so yes, assuming that the provisioner is human or ghoul and not, say, a Mr. Handy.
Giving them mining helmets is a fairly common thing so they stand out more at night.
