I've got the same issue with V.A.T.S. It has really messed up the imersion for me, since after playing for say 2 or 3 hours without sleeping I'll use V.A.T.S and the game freezes, then crashes.
This has forced me to play in a way where I'll at times sleep for an hour somewhere, even though I don't want to lose adrenaline or other perks, because I am afraid that V.A.T.S may freeze the game for me... really irritating bug this one.
i now rely on one of the nexus mods to save anywhere, so i can save before a fight that might freeze me
Seems to happen when there would be a reload weapon sequence needed during vats.
if i'm careful to reload before entering vats i dont seem to crash, but if i havent been paying attention to how close to a reload i am, such as when fighting a group of ghouls, that seems to trigger it
the bug might not be the reload, maybe its just too many targets for vats to track that crashes the game as the two seem to go hand in hand when it happens

This mirrors my experience. I've only run into it while entering VATS for the first time in an encounter (and usually an encounter against just one opponents), but between it and the newly increased time between saves (because of needing to sleep to do so), I've played much less Fallout 4 than I otherwise would have because of lost progress.
I undertand that someone would play less because of this issue. I have logged 5d 2h on my current survival mode character, however it is most likely more in the range of 6 days becuse of loosing 2-3 hours, and I am talking about real hours here, not in-game hours, so many times... soooooo many times... soooooooooooooooooooooooooooooooo many times. :(
All it takes is a few herbs, caffeine and patience ^_^
Unless you want to install a mod? yes there is a way. I can manual save and enable console whenever
That's not really a solution because the game doesn't stop crashing. It just reduces the wasted time, and it's bad because of survival mode sleep-save philosophy.
