2. Maybe VATS won't open if you don't have enough AP for an attack. I know it won't open if there aren't any enemies nearby, but you can hold the button to force it to open.
You didn't ask about it, but there is a critical function in VATS. Every hit will charge the crit bar, and when it is full you can press space to make an attack crit, which does additional damage and guarantees the attack will hit.
You can disable the fps limit in the ini files if you want to. There isn't a setting in options because if FPS goes too high in this game things start breaking.
What in the ♥♥♥♥ does Germany have to do with the limit of violence in a video game? That is like saying my apple juice tastes bad because oranges grow in Florida.
Games released in some countries must conform to more rigorous standards in order to avoid a ban.
Quite a few more violent games have been banned in Germany (and other countries) over the years, resulting in localized low gore versions or no game at all. Some companies have blamed reduced gore overall on high localization costs to hit certain markets, although Bethesda has made no such claim for FO4.
I doubt that was the case in Fallout, seeing as Doom got a full release and Bethesda seems to have a good handle on getting it past censors at this point.
It is possible, but more likely enhanced gore was simply a lower priority than other systems.
