Far Harbor - Can't start Hull Breach 2

Hi, I've read numerous reports of this quest glitching. On my first playthough (non-survival), it triggered correctly. It can trigger in Living on the Edge, Rite of Passage or Turning the Tide. But on my 2nd playthough (survival), nothing I do makes this quest trigger. Killing Machete Mike is one reason for it to glitch, but surely not the only one. Of course, I kept him alive.

The most strange thing is that I can't forcibly start the quest (setstage 0300aec4 100), nothing happens (had to mod survival to test this). So, this thing is heavily glitched. I tested so many things that I can't even describe. If someone has some magical formula to this quest to trigger, I'd like to hear.
