I found an easy way to tell if there ghouls ( the ones that are laying down...not already walking ) around by going into sneak and you'll see it flicker between hidden and detected ( green not red ). Like if a friendly detected you.
The only time they ever startled me was when I first encoutered them, and that was only the ones that rose up and attacked.
Are you talking about the Dragur. Those guys were hilarious. Always stepping into their own traps.
I will never forget on my first playthrough I was being chased by 4 Dragurs. While I retreated, one of them stepped on a pressure plate and all 4 of them got flattened by a spiked wall that came flying out. It was awesome.
The normal Ghouls in 3 and New Vegas had more of a..Ghoulish texture to them. You saw scabs, discolored skin, veiny, raspy voice, patchy hair and is was constantly implied that they reeked. Most of the normal Ghouls in 4 just look like they have wrinkly skin.
There are a few places with more ghouls around, The Slog immediately coming to mind as an example.
But then you play the game long enough and realise that they only appear in specific, scripted areas, and it's less scary or entertaining and more annoying because it wastes your time when you have to literally wait for them to drop in/down enough for you to be able to start hurting them.
Though it does give you time to leave them a surprise, which is always nice I suppose.
