Fatigue Penalty, Survival Mode
Not only can you only save at a bed in Survival mode, but the game penalyzes you if you only take a one hour catnap. Do that a few times, and you are penalyzed with the fatigue syndrome, which requires you to sleep more often. Sleeping for one hour, then getting up to see if the mierable weather has cleared yet doen't count as rest.

I tried that recently. There was a heavy fog, so I slept an hour, in a clean bed, got up to see if the fog had cleared. It hadn't so I went back to sleep for another hour. Then I did it again 4 more times before the fog finally cleared.

At the end of thi time, the game decreed that I was exhausted. I've been in bed for 6 hours, opening my eyes briefly to see if the weather's improved, and I'm exhausted. It's as though the game only wants you to save once a day, and, if you "cheat" by only sleeping an hour, it throws a tantrum and fixes you with the fatigue penalty.

I guess the relevant question here is, "How long must you sleep every time you save to avoid being penalyzed? Four hours? Six hours? Or must you sleep a straight eight every time?

I typically sleep for 8 and only seem to get lethargy or fatigue when I take a 1 hour nap which I cancel out of in order to save more quickly. Though diseases are really unthreatening since doctors are plentiful and Chemist 1 is op.
Don't do it like that then, problem solved

I think the sleep only at night mod is good to counteract this, as it allows you to auto-save entering a new area as well, meaning you won't feel pressure to take those 1 hour naps in the first place

i think it is like 6 hours, but i really don't keep track. I treat injuries and other penalties like i do in real life. I often go to work on 4 hours of sleep, or might have a sore foot, or maybe a cold. I am never at 100%. I still get the job done. If you play the mode enough, and correctly, the minor penalties don't ratttle you too much. It is part of the immersive experience. Now about the save system: it seems bad until you start playing the mode. Last night i cleared out the brewery over by Oberland settlement. Inside the brewery there were 5 mattresses. Or take the area near CIT. On the one end there is a bed. And near to this there is a mattress. And less than a minute away there are two mattresses on boats in the river. So the mattress situation is more of a mental problem than a game play problem.

Generally sleep for 1 hour for saves, usually 3-4 of those then an 8 hour sleep. Seems to balance out, I carry lots of Antibiotics and have clinics set up in most of my settlelments

and take up the first chem perk too. It allows you to make stimpacks and antibiotics. Another great thing they did with survival, finding uses out of some previously useless perks (chem here, and the lead belly perk).

if you can get to later part of game. having gone through the diffcutly will make it that much more sattisfiying. you tend to have to deal with infection and disease less as you level up.

On a sleeping bag sleep 3 hours, on a mattress sleep 5 hours, and in a bed sleep for 8 hours.

You get sleeping sickness or narcalepsy.
