Am I turning into a ghoul?

its a glitch.

You can only turn into a ghoul with command: "player.setrace Ghoul"

Since Ghoul is entirely different race (Like in skyrim theres Nords, Elves, etc) Ghouls and Humans are along with Supermutants different races in the game, and work just like that.

Above command works but you need to re-start the game for the neck to be fixed and visit a plastic surgeon to "push back" the nose to fit ghoulish face texture. (yes, its an ingame thing.)

DONT USE "player.setrace Supermutant" because it might break your save.

I was not serious, it's that dang "blackface glitch", I don't know if there's a fix for that. It happens sometimes, but usually my face is perfectly normal.

So, I believe you legitimately can start turning, somehow or another, as my character looks less human than prior to my latest venture. I wish I could add a picture here, but yeah, my guy is looking rather Ghoulish and I don't have any mods that would make this happen. Also, since they Ghoulish perk apparently DOESN'T turn you, then I'm left to assume that it is indeed an in game thing. The only mod I have that even messes with anything body or skin related, is NPCs Aren't Cartoons, which doesn't affect the player.

It is 100% a mod. But since you won't show an image, no one can say whether it's just a texture error. In the vanilla game there is no mechanism to turn into a ghoul. There is the perk, but that's only very specific use.
