Report a translation problem
You can only turn into a ghoul with command: "player.setrace Ghoul"
Since Ghoul is entirely different race (Like in skyrim theres Nords, Elves, etc) Ghouls and Humans are along with Supermutants different races in the game, and work just like that.
Above command works but you need to re-start the game for the neck to be fixed and visit a plastic surgeon to "push back" the nose to fit ghoulish face texture. (yes, its an ingame thing.)
DONT USE "player.setrace Supermutant" because it might break your save.

It is 100% a mod. But since you won't show an image, no one can say whether it's just a texture error. In the vanilla game there is no mechanism to turn into a ghoul. There is the perk, but that's only very specific use.
