Done, you beat the game.
I don't think any SPECIAL benefit from a sneak build...so any SPECIAL will do. Maybe just Agility is important if you use vats
Start from those perks I mentioned: sneak, ninja, gun nut. With gun nut you can tune weapons and add silencers. Use Scrounger early on, it's an overall great perk for any playthough.
Other nice perks to have for that are: Mister sandman, Action boy, Rifleman/Commando/Gunslinger, Idiot Savant, Strong Back, Lone Wanderer/Inspirational, Locksmith/Hacker, Pickpocket.
the specials can be changed depending if your gonna use vats or not if your dont use vats then you only need 4 in perception for rifleman and you can put distribute the remaining points into charisma intellegence or lucks for stuff
perks are easy anything to increase damage out put Rifleman, mister sandman, ninja, sneak, life giver, armorer, gun nut and probably the most important perk ever lone wanderer which is probably the frist thing id get
Chemist Rank 1 should be the very first perk you take. This will enable you to make Antibiotics. (i'm not sure that is obvious, but it is a requirement)
Being able to make them yourself will take the pressure off not having access to doctors early in the game. Since infections (easily caught from molerats, bugs and ghouls) will kill you with periodic damage it is important to be able to treat these yourself. Later you'll be able to purchase antibiotics, but in the beginning it is a big help. Every other illness is survivable, but infections will kill you if not treated promptly.
Otherwise it's just like any other stealth build. Get Armoror / Gun Nut to mod your weapons and armor. Choose those perks like Stealth, Sandman and Ninja to increase damage while stealthed and/or with silenced weapons.
Make a run to Drumlin Diner and purchased the silenced pipe rifile, this will give you a silenced weapon much earlier than you can make yourself.
Another thing. Radiation. Using Radaway will make you susceptible to infections. So you can't woof them down in Survival. You can take Lead Belly to decrease the rads you get from irradiated food. This is helpfull in early survival.
Bottles. Bottles are used to fill with both dirty and clean water. Collect every bottle you can find. There are 8 in Vault 101. If you plan to do settlements and cook the food they produce, save empty bottles to fill with Dirty Water. DW is required for Noodle Cups and Veggie Soup and these will be your most efficient food items as they satisfy both hunger and thirst. But if you fill all your bottles with Clean Water you won't have the ability to collect Dirty Water to use for cooking. Dirty Water will be the limiting factor in your ability to make soups.
Cook all the meat you collect. This removes the rads.
For a complete tutorial on illnesses take a look at this. He does a 30 hubflower run around Sanctuary (a valuable thing in and of itself.) During that run he discusses illnesses and his experience while experimenting with Survival. I found the info invaluable before my first run.

Edit: The hubflower run is #4 but the first 3 have a lot of Survival tips too.
I would add get some of the endurance perks too. I always take Aquamarine. There are a lot of radiated waterways in the world. The easiest way to get to Diamond city early game is to take the waterways from Concord. And enemies will not follow you into water, or cross it. And i would pick up toughness and life giver. Life giver looks prettty lame. And it is at the start. But rank 3 gives a health regeneration like Solar powered, except a much lower level.
Gettting gunnut will help make those piece of crap weapons killer weapons. I always pick it up. With this perk, you can eventually put suppressor on just about everything.
if you are doing settlements, get local leader perk under charisma. It is 6 charisma, but a must if you are planning on building settlements. Settlement building and survival seem to go hand in hand. When you kill enemies in an area, they stay dead for a long time. So you can clear an area of enemies and loot and then go to the next settlement. All those fans and cans you found will have their resources available for building at any connected settlement.
Vats makes me feel overpowered. I only use it if I get overwhelmed or near death.
