Love sitting at 99% 83/84 and unable to put this game to rest thanks to bugs.
So I went ahead and found the item code and used player.placeatme 06044312 1 to put it around where it was supposed to be. I picked it up and got the fancy item pickup screen, but I still cannot craft the item.
I took a look in the creation kit, and found that the requirement for the creation of Nuka-Extreme is for an invisible quest to be set to stage 40.
Entering the command "SetStage DLC04NukaSodas 40" alowed me to craft the Nuka-Xtreme.
I feel slightly bad for having to use the console to get the final achievement, but since the thing's bugged, I didn't see another way.
Cycled through stages up to 200, got it. Thanks Squid.

I could not find the
Nuka-Cooler Recipe...
I looked up the location and it simply was not there. The way to get it "properly" is to tcl (noclip, via console) under the shelf. I then found it laying under some terrain, so it basically fell through the map for me. Picking up the recipe, brought up the special item screen, and the recipe was then properly added the the mixer, letting me brew it.
Ah! So that's probably why we didn't find it in the first place.
Only the Mixes because you mix them in a plain bottle
other regular colas look quite colorful ... looking at them makes me want to go buy a soft drink
