Would you recommend Horizon or Lunar Fallout for a new Survival playthrough?
Opinion? Any 'realism' mod to add also?

Regardless of what you might choose:

Fully read the compatibility issues and bug reports for ALL of the mods of this scope that you want to try. Some may be completely fine being installed over each other, while others have distinct issues that cause crashed and bugs and more.

Good luck on Survival either way!

Horizon is definitely an increase in difficulty, mainly via increased complexity. For example creating ammo can be a tedious process. I recommend taking a look at its wiki for more info.

My view is that all the big mods that amend gameplay have their quirks. You either play them as-presented or not at all. Mixing mods can lead to major problems.

Pretty sure that Bethesda/Microsoft can turn any good idea into a complete GNAFU. At least Bethesda gives us a way to fix the game into something worth playing more than once.

Neither. Both are incredibly buggy and require too many patches for compatibility.. Just grab IAF(check out the "IAF patch repository" and its requirements for more fun stuff), MAIM, SCOURGE, and Advance Needs. Tweak the game to your liking without compatibility issues. All these mods work together in harmony

IAF and the patch repository add in first person animations for items from other mods. Works great with the chem animation mod. Check out the repository for what mods it patches... all of them are cool

MAIM adds in location damage/bleeding through perks, so no scripts and won't tank your game. Makes combat feel more stressful. I should note: it does overhaul the chems in the game(just the effectiveness of them, as well as changing their names) to coincide with the gameplay effects.

SCOURGE is a enemy health pool slider mod. Allows you to de-level list the NPC's, and you can then set a certain threshold for what kind of health they can spawn with. Good for more realistic and random encounters.

Advanced Needs is like survival mode on steroids. It comes with A LOT of things the base survival mode doesn't, and easily outdoes most other survival/realism mods.

milkyway, I sure wish I could chat with you somehow. I am using this advice you gave as a base foundation for a new build. I have an existing build that I have been struggling with deciding what I should keep and what I shouldn't keep with these as I am not sure adsactly what I am able to use with these and how the load order should be for these mods to work together. Was just hoping I could get a quick screenshot of your modlist as just based off of this advice you gave you seem to really know what your talking about. As, thats how I found this comment after doing significant research on which modes needed to perfect this build. I sent a friend request just so I could reach you and ask you for a quick modlist screenshot. Would be much appreciated. Thanks.

Generally speaking if you're ever confused about the load order of a mod on Nexus, *ASK* on the mod's posts. The forum posts are very difficult to search through particularly for large-population lots-of-downloads mods. The worst they can do is say 'well this thread here' and link you info.
