If he gets "lost" when you tell him to wait and then enter a new cell he'll usually spawn where you do. If he's not there then he's not there. If you've assigned him to Sanctuary then he might be in the dog houses there.
If you have access to console commands you can bring him to you from wherever he may be.
Yes. That was my experience too. Although it can take a few days for him to show up. But I've seen others state that they've dismissed him at Fort Hagen and had him remain there. This is proabably a glitch with that quest.
1) go to sanctuary and put the only dog house and a Clear view, he might Respawn.
2)there is a console command that let you move the player to a Specific person. i lost Preston one time and used it, ended up forgetting that I actually sent him near the fort for an Assault
