I suggest reading the mod page(s) including the Posts and Bugs sections.
You can definitely get it from Garvey/PAM as a radiant quest (or anyone else who gives out radiant settlement quests). Any settlement that doesn't unlock through specific scripted means (so not Mechanist's Lair, Nuka World settlements initially, factional HQs etc) can be included as part of that radiant list, and that includes the majority of Commonwealth settlements including Spectacle-- it says as much on the settlements page. That list on the Taking Point wikia page isn't at all the complete list of settlements. And whilst I *am* using mods, I don't run any that alter or affect Spectacle Island, hostile creatures, settlement beacons, Preston Garvey or any quests at all (I don't have *any* modded quests whatsoever). That and I was able to find an old save from a different FO4 playthrough where Spec Island was completely untouched and not a part of my coterie of settlements and managed to wrangle the gig out of Preston (Took about 6 hours of farting about) and it seemed to work out fine.
I hope you're wrong, but I reckon this is what it might be-- either that or the hostile don't spawn glitch this Island apparently has from time to time. Some of the mirelurks were able to slip away when I took the island a couple of months ago, but this quest notwithstanding, I haven't seen any hostiles spawning on or even near the island. I guess it's time to bust out the diving bell and have a wander around to see if there's anything underwater.
Is the Mirelurk Queen you fight when clearing the island flagged in the radiant quests at all? I don't suppose I could just spawn in a bunch of mirelurks and a queen in and see if that trips the objective?
Spectacle Island has one queen mirelurk, one/two king mirelurk, a couple of the spitting mirelurks(forget their names) and several mirelurks when you first arrive there. It can be opened up by any faction giving out radiant quests or not. Sometimes you will not get any quest to open it.
When the island is attacked they come from the north and via the beach to the west and occasionally one or two from the east below the workbench station. I've never had them spawn next to the workbench station/shack.
Are your defences killing the mirelurk super quickly?
If so try taking the defences down and letting the mirelurk attack and you kill it. See what happens.
I've opened spectacle without any radiant quest and it made no difference. The island has always opened up for myself after killing the mirelurk queen and activating the power.
However, search the island very carefully, especially to the north.
