I think they were supposed to also change from their slave clothes into normal clothes possibly the "certain armor items" Bored Peon mentioned.
The developers never really put any thought into NOT allowing NPCs to come up with those certain combinations to cause that conflict and make them walk around in their underwear.
Based on what I've seen by messing around with NPC equipment via the console, I 100% would not be surprised if that was also the case.
Yeah, seriously. To be honest, a lot of the clothing and armor combination restrictions feel like a case of 'too lazy to fix clipping issues'.
Seriously, did the ghouls go around sewing shirts and pants together for 200 years? Is that what drove them Feral? That's why they went feral isn't it?
I'd always thought naked guards were because of looting their corpse? Then they respawn naked.
If you use the showinventory command on them you will see they still have the clothes.
In fact when I used the console to equip the clothes that command showed they had it removed the nearly invisible leather armor part it showed they were wearing.
