You can either just skip the vanilla intro, or you can start from multiple options that have nothing to do with a pre-war life in Sanctuary.
Good thing is that this mod also edits some dialogue options that else would be a bit awkward.
Regarding survival I usually recommend "Advanced Needs II" by Flashy JoeR...
but this is no longer available in public (not in an easy way, but you may try your luck in the internet), and I haven't tried others.

These provide a complete rebalancing of the game (especially loot) that really helps with the feel of Survival mode. I only use Unlimited Survival to enable saving without having to sleep; I always felt that was a tad TOO restrictive. I don't use fast travel at all, because that is just a cheat in my mind.
