What's the difference between CBBE and Vanilla Body

CBBE isn't that great to use vanilla. CBBE + Bodyslide = perfection. You can set body shapes, sizes, etc to whatever you want. Personally, I would suggest CBBE + Bodyslide + texture/mesh or whatever the hell they're called of your choice. Just to provide a more realistic look. Or you can live out your pervy fantasies if you like. The underwear options are pretty nice too if you prefer a less smutty environment for your Sole Survivor.

I would mainly do underwear
I wish there was a mod so Under Wear was a Clothing Item and I'm not joking when I say for Immersion
It'd be also cool if there was a mod that randomized Boob Size mix that with a height randomize Because it's unrealisitic that everyone is a C or DD

Actually, there are. Mainly for women though.
Go for Vanilla, or Enhanced Vanilla, but CBBE has settings in the Body Slide for all these things. Watch Gopher's Vid on fallout 4 CBBE.
As for random, there's a couple but they don't work the best and are not maintained.

Yeah. One thing I learned how to do, and it's simple and easy (not insanely complex and horrifying), is to change all the clothing over to my bodyslide setup. Takes 2 seconds, little longer maybe, but you can do it so easily and it makes the game so much better that way. There's a lot of outfits that are set to specific presets that I find distasteful personally, but I love the outfits. Using Bodyslide to make them conform to my own personal likes = win win win.
