I think that's going to be 'harder' to do, since Vaults aren't said to have been made wordwide.
Yeah, they could make a game around other countries, but Vaults are kind of a thing, and seem to be a US deal.
Now, if they wanted to make 'something' that other countries may have done/tried, that could be done, but they'd probably have to come up with a reason why. As it stands now, I don't think other countires bothered.
And from a tongue in cheek perspective the 50s idealized Americana and its oddly stunted yet advanced technology is really what makes Fallout unique from other post apocalyptic games.
Second, China would be the likely alternative setting. The reason is simply that is the other place that had SOMETHING in place to protect SOME of its citizens. As far as I know, Vaults are a US/Allies thing. Vault Tec simply has a bunch of back door deals with the US Government to use its facilities to conduct radical experiments that would otherwise violate human rights (even in the Fallout world).
China had some similar government run bunker based on the things you learn in other Fallout games. Rather than create a world of crazy scientific anomalies (Super Mutants, Deathclaws, and heroic vault dwellers - but also good things like GECKS) they end up with some kind of weird shadow government and continued governmental corruption and backstabbing.
The US has a similar group (The Enclave), which is basically the remaining governmental form of the US.
So yeah, long story short is China (PRC) and the US (Enclave) are the only two "countries" that seem to be left after the war. As far as I can remember no other power is ever mentioned as having operatives still active (No Russians for instance, nor UK/EU etc).

One example is Boston itself. It wasnt hit with a direct blast but the apparently larger than normal nukes that fell nearby made the glowing sea and stillanaged to kill a crap ton of people and wreck ruin.
Its implied the UK and Ireland have a population. Both Alistair Tenpenny and Moriarty's parents somehow traveled from there to the capital wasteland.
China is implied to have had underground bunkers and a population.
Canada and Maxico have a few settlements too presumably.
One interesting fact I have found is the globes show the Mediterainean Sea as a landmass... so Europe and Africa are one large continent now.
Oh and aliens exist canonically.
