I am doing my 2nd vanilla play-through right now with no mods so that i can get the rest of the steam achievements i missed on the first time around. so far i'm level 42 and having a blast :)
In some settlement for example the NPCs are running through the buildings or don't fullfill their job as intended. That's cause I placed a building on former farmland where corn was cultivated. So, even though there stands now a bigger, double-level building, the NPCs still act as farmers.
Some people also have issues with crashes. I mainly had it in the first 50-100 hours.
The game also loads every object onto the map, which you've ever dropped onto the map. That can be a problem, cause your character can only carry a certain weight-limit and loot has different "kilo-prizes". Some loot is simply more valuable.
So, the more junk you drop onto the map, the more the program has to load into the screen.
From my experience: every time you leave a building I've to wait 30-300 seconds (can really vary a lot) that the screen loads. That's at least my experience. And I'm lvl 90+
You're asking the wrong person. I'm not using mods. If a developer wants me to play a proper game, then the developer has to fix this.
Should you? That's a matter for debate.
Personally, I'd say modding is like an addiction. You can do just fine without it, but once you get into it, you're not going back.
Haven't played a Bethesda RPG without mods except when my PC died and I replayed Oblivion and FO3 on the PS3.
So yeah, addiction, sure, but it's more like "willfully choosing the preferable option."

even removed the DLC s .. only vanilla noew .. and feeling much better now .
i had a lot of fun with the following mods:
- better (power) generators
- better junk fences
- better settlers (v.2)
- boss chests contain legendaries
- brighter settlement lights
- stackable concrete blocks (foundations)
- easy hacking & easy lockpicking
- faster terminal displays
- homemaker (adds more building objects)
- increased settler population
- longer power lines
- lots more settlers and enemies
- unofficial bugfix & patch
- scrap everything (from settlements)
- very rich merchants
- weightless junk
- unlimited settlement objects
- item cheats
and my mod list was a lot longer at one point but these are the ones i kept back and remember being glad i was using them. i built up these settlements with towers of glass buildings at each site, elevators, each place all lit up. it was glorious. so yes, you can have fun with mods too, and they're really not that hard to install and get into.
that's what i recommend cos there are so many mods that fix stupid features in the game
But that's not the case with FO4, the game works without mods very well, might contain some bugs, but what game is perfect? I say none. Mods do help with that, sure, but aren't really necessary in that regard. Even unmoded FO4 is playable. That being said, it's a Bethesda game, so mods are a must here. Mainly character beautification mods, because let's be honest, Bethesda's characters are ugly. That's how it is. Then you go with some QoL mods, because the game mechanics isn't the most attractive out there. And then you add some companion framework, because vanilla companions suck. Having just one companion with you? Whose ingenious idea was that? And the list goes on. XD In the end you can have a lot of mods just to adjust the game to your taste. Vanilla FO4 is playable, but modded FO4 is way better. That's an old truth.
