No Sound Bug!
Okay I got a beastly PC but i put my game on high only so lip sync is normal. I played last week on Thursday and Sound was working.
Today the intro to the special cartoon sounds alright, no issues but then when it loads the menu there is no SFX. I load a game and all SFX are mute. Guns, and speech is dead silent. I looked in settings and its all high in audio.
is there something i am missing?

Check that windows 10 still lists the correct sound output device. Fallout 4 has a tendency to mute the audio if it thinks the speakers got unplugged as well as sometimes changing to HDMI audio for some reason. Unless of course you have an HDMI audio device/monitor with audio this will appear as muted sound.

There was one instance reported where someone used wireless headphones and had to move them to another USB port due to some RF interference from the video card causing random disconnects that were only noticeable because of fallout 4s decision to mute the audio if the speakers appear to be disconnected even for a period so short it would otherwise not be noticed.

This happens to me if I have earphones plugged in the front panel and want to use earphones in the back of the PC. If I start the game with both earphones connected, audio goes to the front ones; if I just disconnect the front, the intro video briefly starts to sound but interface and general fx is muted.

To address the issue (in that case) I need to close the game and start it with nothing plugged to the front panel (as I want audio from the back jack).

In the sound settings on my system I have to actually set it for what port the speakers are plugged into or did before installing a monitor with hdmi sound. The game or windows 10 will sometimes incorectly set the sound device to something not present or not having speakers.

I also have to set the frequency for the quality if we set it to high fallout 4 will not recognize the speakers or it will hae other sound issues.

So whats' the fix for this?
Fallout 4 and Starfiled only games do that (and FO4 even plays audio in the video intro,m but not otherwise).
There is zero support from bethesda or community. Actually,bethesd _dateleted_all relevant info to this audio bug from their tech support website, leaving some inane remark releavant only to Win11 about audio equalizing.

Nope, Doesn't work. Windows 10 v. 22H2.
First thing I tried actually, after rechecking configuration and disabling everything I use.

Stupid if that was so, because game specifically designed with Surround\Spatial library they killed hundreds of hours and thousands of dollars on.

Next was
1) attept to set 16bit 48kHZ.
2) Running in windowed mode (full-screen breaks HDMI output).
3) Reconfiguring and setting to HDMI (as we are limited to stereo, why ot?)
4) Resetting game config
5) Reinstalling.
6) Fixing Windows image (you cannot reinstall DirectX on win10 this late, it's part of OS)
7) Updating sound drivers.

Also attempted to use Voicemeeter to avoid issues with game being idiotically sensitive to stability of device (if connection breaks, game disables sound forever, means that if you have wireless output or bad contacts, you're screwed). Nope. It appears according to Voicemeeter, that game switches sound off. Note: voicemeeter is known to work with this game engine.

Enabled debugging in ini file and marveled at two hundreds of errors caused by _default_ game content's scripts. But that mstly content-related errors, nothing lethal there.

Oh,and cherry on top, Fallout 3NV developed by other studio works perfectly, Fallout 3, developed by BGS, shows same issues.

however the last part you may have to go into your fallout folder and find the directX.exe file and reinstall it. then restart your pc.

Can confirm that, Fallout seems to run up to 32 Bit 192 KHz, but if you use 384 KHz, it's no sound.
Usually it's best to use 32 Bit (or what's highest on your soundcard) and a maximum of 96 KHz, else there will be problems like lags, no sound or even crashes.

SOLUTION!!!
FirtsI tried everything in all the posts on steam on Reddit and on Bethesda community and nothing worked for me it's not fair from the devs part because it isn't such an old game and they should have kept updating at least to optimize for Windows 11.
So I have an Rog Ally and just got 3 months of game pass for free and saw fallout 4 downloaded it and everything worked perfect first time but because It didn't have the dlc and they were asking about 30 euros for the goty I decided to buy the key for 10 euros online but when I installed via steam surprise.... No in-game sound past de intro animation so here is what worked for me I sow a video on yt in Portuguese of someone downloading and installing a dx9 aditional add-ons I installed it my self and boom works perfectly now I will leave here the download link from mega it's still working to this day and if not contact me and I will send you the archive just download execute and install as easy as that

For me, it works when i close all my Sound-Software like Steelseries Sonar, and play only with the onboard audio stuff =)

Figured it out.... At least for myself. I have steelseries Software. I can change the outputs through there. Well when i changed it in the windows task bar to the audio source i wanted the audio then came back. Check and make sure you have no 3rd party apps controlling audio
