That is:
1. plug a headset/speakers in the rear jack
2. start the game
3. plug another set in the front jack
So, it is a bit strange you are getting this issue even without replugging audio hardware after the game starts.
Try some things to eliminate "alternative audio routes" if possible. If you are using the front jack, unplug anything from rear jack, and vice-versa.
So essentially fallout4 has trouble choosing the right sound output in case you have different output available.
In my case, if I plug both rear and front jacks, I get audio in the rear ones. If I start the game and remove front jack, the back-plugged phones behave the way you described. Restarting the game without fiddling the audio connectors allows me to play with all audio shining.
Hope this lot of information helps you troubleshoot your situation.
This is exactly what happens in the scenarios I described in the last post.
So, you have something "switching audio devices" after the game starts. Are you by chance using bluetooth earbuds?
Your headphones then don't have 3.5mm plug, but a USB connector? If so, try something you may have lying around in 3.5mm stereo (TRS) plug and chug it in one of the PCs audio output jacks. At least to see if it works like that.
I doubt this is the case, but if you are using a TRRS jack (cellphone 3.5mm, that carries also microphone signal) and plug to a TRS (ordinary stereo) 3.5mm connector, the sound would overall will be "washed out". But if that was the case cinematics' audio would be very weird too. And you'd be listening to all audio, but "washed out" of sorts.
You should try the most basic scenario, the game is really buggy when it comes to audio dynamics (yet, it is amazing when it works lol). Well, game is very buggy in anything, and Bethesda gives it a .. well. not even a dime it seems. :P
I mean, if at least you can isolate the problem, other routes towards the fix may be revealed.
