This is the first time it has crashed for me, to the point where it has force quit the game but still
I had the same thing happen to me, and he had Fizzy Lifting Beers and the recipe for the same on him when I checked

I've just had it happen to me and because of that I googled it to see if it was a bethesda easter egg or something. It actually is related to the Sim Settlements mod. I have a feeling it's an attempt at a "random event" occuring in the world. However the spawn was off or maybe it was intentional. It happened to me in the core of the city so maybe it's to make it seem he fell off a building? Thus the screaming? Because I've seen NPCs fall off of tall places from poor noding or navigation breaking and not scream.
It was really funny to be honest, took me back to the "Mad Wizard" from Morrowind/Dragonborn DLC on Solstheim. I had a similar occurance happen back in Saints Row 2, a man dressed as a sky diver came screaming down from the sky and hit the ground, dying.
Intentional or not, I liked it.
It's like that fizzy soda from Willy Wonka; I guess he either didn't drink enough, or it didn't last as long as he thought it might. Or let out too big of a belch.
