I've recently got Fallout 4, and can't get it to run above 1fps. At the same time I got a new video card, rather the on-board Intel 4000, and that's where I think the problem is - Steam System Info has the Intel 4000 as the video card.

I have:
1) attached the monitor to the GTX 1050 TI
2) disabled Intel 4000 in device manager
3) tried re-installing Fallout 4
4) validated the Fallout 4 files
5) tried varies 'fixes' to the ini files
6) did a clean re-install of the GTX 1050 TI files
6) have an up to date driver for GTX 1050 TI
7) optimised Fallout 4 with GeForce Experience

What do I need to do to get it to work?

First things first. Click Steam | Help | System Information and post that here. I'd like to see what Steam says about your hardware and drivers. This may shed some light on the issue.

Second, you tried some fixes then ran Geforce Experience. To ensure you have a clean set of ini files you should delete them, they are in Documents\My Games\Fallout4 then launch the game to create new ones then re-run GFE to update them. This alone may solve your problem but go ahead and post that System Inforamation just in case.

I've deleted the ini files (a few times), I think they are now altered by GeForce (and include the line 'sD3DDevice="NVIDIA GeForce GTX 1050 Ti"'

your Primary Display Resolution should be set to your "native" Desktop resolution for testing.

Device Manager has GTX 1050Ti driver 22.21.13.8476, with the date of 27 June 2017. The 21 Dec 2015 date is from the Intel 4000 driver; I don't know why it shows like that.

So I did a clean re-install of the GTX 1050 Ti drivers, via GeForce, Steam system info had:

and that's what shows with Intel graphics being disabled, and a couple of clean re-installs later.

When I use the GeForce optimised settings i have to alter the settinngs to windowed and borderless, otherwise it crashes. The title screen, the opening movie, and the game selection screens work OK - just the game that doesn't.

What do you mean about the resolution thing - I'm not clear about what needs changing?

Can you set your Primary and desktop resolution to 1920X1080? Be nice to know your native desktop resolution. You may google it.

if you arent going to use the Intel HD, go into the BIOS and disable there. then neither steam, your game, nor the OS sees it. I have an Intel HD on my motherboard and an Nvidia card:

Fallout 4's launcher has a very specific driver name inspection built into it, It'll grab your GPU name and then assign settings based on what class it is.
However, Fallout 4 came out before the 1000 series gtx cards, so It'll say that It's not Identified regardless.

But considering your fps, make sure to assign the 1050ti as the primary card via the nvidia control panel, and do the above.

Set up your driver and ensure GPU is selected through the nvidia graphics control panel as per nvidia's website's instructions. Delete your ini files and relaunch.

That image from techadvisor is useful - I can now see what people meant when they said 'set it to high performance NVIDIA processor': I don't have that option - it's not in the Global Settings boxn and in Program Settings there's only

1) select a program to customise, and
2) specify the settings for this program.

So that's the new problem - how do I get that option in the NVIDIA Contol Panel?

most motherboards have a pcie/16 and a pcie/8 slots. from the nvidia installation manual:
"Note: It is recommended that you install the graphics card into the primary PCI
Express x16 slot. If you install the graphics card into the secondary slot, your system
may not recognize the graphics card and there will be nothing displayed on the
monitor."

mine says Primary Bus: PCI Express 16x
yours says Primary Bus: PCI Express 8x

try moving your card to the 16x slot. may not make any difference but worth a shot.

Thanks for everyone's help - I was assuring myself that the GPU was in the right PCI slot, and downloaded a program (HWiNFO) that confirmed the Intel graphics was still being used, despite being disabled, so I went into the GUI to stop it.

So Steam system info now shows Nvidia. I also moved Fallout to another drive, and despite the GPU still not being recognised, I am up and runiing in high/ultra. So got here in the end.
