Report a translation problem
I believe pretty correct. Every time Windows up grades (ver. 1903 now), it sets my sound system to Dolby Surround which I don't have. I have to control panel the sound setup back to 2 front speaker stereo and then things work correctly. Just may be your problem.
Like I said though, this audio issue actually showed up in other games and not just Fallout 4. !00% clear in the front, muffled to the left and right and not being able to hear anything behind you. I had this issue appear in games like L4D and Dead By Daylight.
My current fix was to check the box in my speakers properties in the Enhancements tab to turn off all audio enhancements despite none of them being on in the first place.
If you're using a realtek integrated chip look up what chip you have, download the latest driver, disable the device in device manager and install the new driver.
Play with Exclusive mode. I got my behind me sounds back by unticking "give exclusive mode applications priority". Welcome to Windows 11 :(
