* Karma & Crime (from same guy as Advanced Needs)
* Monsterous Monsters?
I highly advice to NOT set your Hp to 1.

Ok, maybe extravagated, but I hope you get my drift.
I play on Survival, with a hotkey for quick save (because crashes, mod conflicts and random issues).
Balanced Survival Quicksave.
Instead of not being able to die (one life), i actually set my game to be very difficult, require strategy and i die a lot... Sometimes even going through a simple place turns to be a difficult task...
Mods i recommend:
Better Locational Damage (overhaul of health and damage, adds headshot feature, adds bleeding, and a lot more).
Agony (crippled limbs require surgery, etc).
PIRAD (total radiation overhaul, with sickness from ingested rads - like from food).
Pack Attack NPC Edition (must have, changes NPC AI).
Food Mod (removes healing from foods)
Crippled limbs fix (i don't really remember if Agony prevents this already)
Fallout 4 Loot Overhaul
Lower Chance of Finding Stimpacks Radaway [...]
Ghoulish Ghouls..
I use a lot of mods and i made patches for everything i use, but take a look at these if you're interested (:
Don't know what it's about but you could look into that.
Not sure it's compatible with survival to my knowledge it does the same stuff. I used it 3 hours and then quit far to tedious for me. What it does is you play on 1 level below survival and this makes it a customisable survival mode
The mod Survival Options will allow saves and to tweak many survival aspects.
Yea, this game is intentionally way too easy.
A pipegun replacer may as well be an official update as well and there is a sim mod for this, "Yona self defence guns" i think its called. This is really great because upgraded they are awsome for the player, but also for the enemy Npc's and also allies.
"Feral ghoul Outbreak" adds hordes down town and may as well be official to and it isnt to demanding at all.
"True Storms" very sim again and max settings adds an excellent layer of difficulty.
"More enemies" or something like that is also excellent. This just adds to the spawn numbers everywhere and is very sim and wouldnt even know its a mod.
Playing iron man does take willpower but it can be done in Fallout 4 and the game may be made to play like that originally. Mines are the challenge with this and just not going anywhere near mini Nukes early game and knowing where they are in the game through trial and error and alot of restarts :)
Mutltiple companions is also a must and thats why there are so many power armor colors and things i think. But got to add no power armor damage and no ammo use keywords to a mod in the CK to each of the companions, Nick and Piper and such, and just makes them like Danse and settlers.
All these mods may have been official if not for consoles and no kidding, survivial wasnt added to the game until there were sim fast travel mods so i really think some mods were meant to be official.
Noir Penthouse, backpacks, modern furniture and other official story quest and equipment CC items also highly recommended.

with this you can adjust the damage etc. to make the game as dificult as you want.

With this based on how you set it up and the karma you get you can have random raider or bounty hunter attacks happen etc.
with those three (if they work together have not actually tried the first one with the other two yet) you can create a game mode that is as difficult or as easy as you wish.
You will not find any permadeath mod, I tried it. But you dont want to loss your run for car bug instant kills or game bugs that kill you. Anyway there is not any permadeath mods available i any place.
If you want to make this game a challenge, do the next to your own risk. I do it because I had plenity of time and the game seemed horribly easy to me. I repeat, do this to your own risk.
First of all, I recommend you to add the mod that add the Black guys coffin music when you die because you will die a lot, specially in the early/mid stages.
Water travels and institute teleportation are the best way to play this mod combination.
What you need to do is the next:
To make the game a really challenge, try to install a ♥♥♥♥♥♥♥♥ of mods until any unit oneshoot kill you:
- Add War of the Commonwealth, so the map is full of enemies.
- Add Arbitration, to improve enemies AI and change both you and your enemies damage.
- Add the Travel NPC groups mod
- Add the mod that makes the enemies the same level that you have or above.
- Add the mods that add more enemies in the map and increase enemies number in cities and places
- Add the mod that makes the Glowing Sea a warzone and the Sentinel Site an impossible challenge.
- Add the mod that make ghouls stronger and add tons of them in lots of locations
- Add the mod that made the cellars reset faster
- Add the mod that makes supermutants way stronger, there is a mod that multiplies all Supermutants atributes like x6 (At this point, you will need to have both heavy weapon and explosive perks at level 5 and you will have to shoot like 6 rockets to kill one single supermutant)
- Add the mod that made radiation way dangerous
- Add the mod that made rad storms stronger and more frecuent
- Add the mod that made Behemots and Deathclaws way stronger and big in size
- Add the mod that add bosses with ridiculous HP numbers. (Combined with the other mods, this bosses are indestructibles, there is just a few ways to kill them, is up to you to find the way after several tests)
What you will get will be:
- Every single supermutant is a super boss with a ♥♥♥♥♥♥♥♥ of health and damage
- Deathclaws and Behemots need at least 20 rockets to be killed
- Assaultrons become the hardest enemy in the game
- You will die in a few shots with every enemy encounter
In this way, you do not need to make your character weaker, like not using power armor or start with stats all in 1. You will play it normally and it will be still almost impossible.
There is ways to made the game even harder. Keep adding mods. The game from level 1-20 is practically impossible and will take you several reloads to do every single fight. Also the game will be so charged in mods that the game reload will take longer.
I tried this experience and the game is very hard even in level 80+
To made this playable, i recommend you to:
- Install a mod that let you freely save the game on survival
- Install a mod that made the ammo weightless (You will need a ♥♥♥♥♥♥♥♥ of it all the time)
- Install a mod that autoreset the traders (You will need to sell items and buy ammo all the time)
- Install a mod that made Junk weightless (Armor and weapons crafting will play a bigger role in this gameplay style)
- Rush the X-01 Quantum power armor at Nuka World.
Best of luck
