One way it might be going off, is if it is combined with the Targeting HUD Modification for Power Armour Helmets - this mod will 'highlight' enemies, but can sometimes trigger on friendly NPCs and friendly Robots for some reason - which causes them to become Hostile.
Thus, combining the two, a friendly NPC that you are just walking up to; can be Highlighted and then Hostile; becoming shocked with the efficient Tesla Coils PA Mod... all resulting in a friendly NPC you are trying to talk to, just dying in front of you.
This may not be 'it' of course, but it is something that happened to me in the past. I just don't run the Targeting HUD and it doesn't happen anymore (for me) - I still run the Tesla Coils once in a while though, heh.

This is what I did to make it work, I posted it on Bethesda official forums and now paste it here. Hope it works for you:
Step by step,
I fast-travelled to the Castle, got back to the NWTC and was able to talk to Harvey. This time, he died after I talked to him, still considered like I killed him. Then I messed with Consol Command ("setessential" kept telling me that the script was not saved, and then I used "xxxnpc movad health 99999" and was able to skip the bug though I tried keeping my distance from Harvey the whole time. I'm not sure what fixed it.
After that, I had no issue talking to him in NW until the end (I did the open season ending just before Shank quest, just FYI but I don't think it matters).

The command is SetEssential xx007D59 1
xx is of course based on which of the DLC you have, assuming you have all of the core DLC, that would be 06 for Nuka-World.
Do the command any time before you get too close to him and then you can talk to him normally. If you then want to kill him then just repeat the command with a 0 replacing the 1.
You can do this in fact for any NPC you don't want to die (or that you do) you just have to use their FormID (or BaseID) not the REF ID. Like if you click on Harvey you will see the number xx007D78 but that's his REF id, his current copy in your game.
