Report a translation problem
For me personally, my system can playback/record/decode audio up to 24 bits at a sample rate of 96 khz, but it's not set to that for video games and watching videos and stuff.
The point is, maybe your system is set to 48 khz and you can't decode higher than 44.1 khz. That would result in hearing no audio.
Right click on your volume button in the system tray and go to playback devices. Then select your proper playback device and then run through the configure setup to make sure it's configured correctly.
Also make sure that all other audio devices are disabled (the ones that you're not using). That's what I do, so things don't get over complicated.
Check in your device manager and make sure proper devices are selected and disable ones that are irrelevant. There could be some kind of driver conflit where your PC gets confused on which one to use.

if so, try deleting the 3 .ini files and let the game generate new one.
Also switched from running in DX12 to DX11 with no results either...sorry man think we are boned
Every other game i have tried is fully function with video and audio, as well as the other Bethesda games I have so it is exclusive to Fallout 4. Any other ideas to try?
