I like to think about how Video games stuff would function in the real world. And I usually cheat for fun. Its fun to randomly spawn 50 super-mutants randomly and watch them murder DC.
The time dilation effect is easy to explain. Time doesn't actually slow down, its just relative, your mind is working with the VATS system to quickly pick targets out of the chaos around you which makes time seem to move slower for you.
The fact that you take much less damage while using VATS can't really be explained though. The only rational explanation would be that the VATS system somehow creates a shield around the user while its active, but energy shields don't exist in the Fallout universe so... its magic.
Actually, some how achievemnts dont get disabled when you use console commands. So yeah they are techincally 'cheated'

It's another case of internal work, though, not something you just slap onto your wrist. But then, the Sole Survivor can use VATS in Vault 111, before they get the Pipboy in the first place.
