If it bugs out on you and doesn't work, don't worry the Perk from him is only -10 Damage from Robots and Devices. Not too great but that's only my personal opinion on the damage reduction amount - I like him as a Companion otherwise!
Hmm that's a pickle... Not sure other than maybe; have you done his little Personal Quest?
I haven't used him a lot after finding so many others, so I haven't even gotten that far with him - does he even have a Personal Quest?
Sadly, this seems to be a bug that has been around and may ultimately not be fixable...

There is a way to check his Affinity Level (out of 1000) with Console Commands, but I haven't found it yet, looking around - it also won't be of much help if you are playing Survival Mode...
