Where's Fallout4Custom.ini for mods?
I can't find it, nor can I find the Launcher setting in Fallout4Prefs.ini. Do I just add the Launcher setting at the bottom? Do I have to run the game launcher first, if not running the actual game via launcher (I intend to run the game via F4se via MO2 to avoid updates screwing with F4SE)

Navigate to your Fallout 4 Folder at the following location "Documents\My Games\Fallout4"
Within this folder you'll find a number of .ini files.
Open (or create, if missing) Fallout4Custom.ini with your favourite text editor (we use Notepad++).
Add the following lines to your Fallout4Custom.ini
[Archive]
bInvalidateOlderFiles=1
sResourceDataDirsFinal=
Save and close Fallout4Custom.ini
That's it, you're ready to get modding! :)

There is no Fallout 4 Folder in My Games. Do I need to run the vanilla launcher once, and if so how can I avoid updating and breaking F4SE?

If you run the vanilla launcher, it should generate the ini files in your documents - fallout 4 folder.
Then you can copy one of the Ini files, rename it to Fallout4Custom.ini, Erase its contents inside and type in

[Archive]
bInvalidateOlderFiles=1
sResourceDataDirsFinal='

Steam properties with fallout 4 Only update when I launch it, and never launch it from steam. Only a F4SE Shortcut youve created etc.

Put steam in offline mode, then run the default launcher. That should create the ini files. After that, always launch the game using the F4SE executable.

Steam was in offline mode, I ran the launcher, allowed it to detect my video hardware and set options accordingly, and then exited the launcher instead of launching the game. Was that correct?

There is now a Fallout 4 folder in My Games, containing Fallout4.ini and Fallout4Prefs.ini, but no Fallout4Custom. So, I just...make it in Notepad?

You could copy one of the ini files and paste it so you get a double one. Etc Fallout4.in(1).
Open the copied file, remove all the text inside and replace it with
[Archive]
bInvalidateOlderFiles=1
sResourceDataDirsFinal=
save the textfile and just rename it afterwards to Fallout4Custom.ini

pro tip if the custom ini is missing do this:
Step 1 go to Documents
Step 2 go to My Games
Step 3 choose Fallout 4
Step 4 copy either of the ini files that are there
NOTE if you have run the game once to at least the Red Rocket and Dogmeat you should have two ini files. Fallout4.ini and Fallout4prefs.
Step 5 open the copy with notpad, select all text, and delete it.
Step 6 add the [Archive] to sResourceDataDirsFinal= (just copy and paste it from this thread)
Step 7 just rename the ini file from the copy to Fallout4Custom.ini
DONE!

Yeah, I just decided to get the game refunded and wait to purchase it until Bethesda stops updating the Creation Cack. Easier that way.
