Hmm. My best guess is that there is something broken in the .FUZ files used in the audio dialog playback.
Side NOTE: The way that Fallout 4 (And most of the BETH. games.) work is that there is a single archive file that holds the actual audio data for a given supported language like English or French or Japanese. The files are stored in the .fuz format. There is a part of the game engine that reads the audio wave form as it is being played back and forces the 3D character model to be animated / manipulated in time with the playback of the audio file thus making it appear that the character is speaking.
Question: Do you have any mods? Mods for changing the dialog options?
Possible Fixes: Delete the 2.5GB file in your Fallout 4 data directory called: " Fallout4 - Voices.ba2 " Use the STEAM verify system to have it re download that 2.5GB file to your P.C. STEAM will download the appropriate Fallout4 - Voices.ba2 for the language version you have selected. (Side note for fun: You can have the ENGLISH version of the game installed and use the JAPANESE / FRENCH / ETC audio file to immerse yourself in the language.)

I launched the game and there was no effect. Nora still has no voice. The subtitles say what she should be saying, but no audio.

Answer: Yes. Many, many. It was after adding mods that Nora's voice disappeared. That is why I put up with it on the first play-through. I knew it was my modding and meddling that caused the problem.
However, I don't believe it is a particular mod because all the mods I had in the first play-through when the voice broke, I had in the second play-through when the voice was working.
It seems to me that the act of adding the mods is what broke the audio.
Anyway, thanks for trying to help. If you come up with anything else to try, please let me know.

I seemed to get a simmilar or same problem, something to do with geting in and out of warer too quickly causes it. However it seems to affect the save file so if you load an earlier save it goeas away. I also found sort of a fix just use the AFF mod and run the player trough a bunch of animations. Tested it a few times
If so I had a similar thing with my modded Skyrim SE, I needed to move a main audio file from the game folder to my C drive then create a link to it and then it worked fine, I followed the info in this thread https://forums.nexusmods.com/index.php?/topic/6575951-skyrim-no-sound-effects-fix/ maybe it will help you also?
