I have a ton of mods installed. No telling which is ♥♥♥♥♥♥♥ things up, though.
I'm not sure what you've done with setting quest stages, but the quest itself doesn't usually have any special consequences other than giving you Cait, so it might actually be feasible to set stages and get a working result. It looks like the quest stages from about 390 through about 510 deal with Cait joining or not, so you could go through, set each stage in order, and see if it starts working. The SQS command can be used to see which stages have already been set by the game.
No guarantees on it working, though. Quests do all sorts of stuff behind the scenes, and Tommy not being visible means something obviously abnormal is happening.
One of the mods I had installed was causing the issue. I disabled all of them and got through the quest without issue, I've no idea what mods caused it, though.
