If you mean outside of vats, you shouldn't be getting forced third person animations with a simple attack.
Oddly enough, after doing some testing, it seems like doing it in third person does not take you out of sneak, however it still attracts more attention than just a regular hit for some reason. Like a normal swing will keep me in hidden even with other people nearby, but a kill move will bring me straight to caution or even danger every time.
So far the only workaround I've found is to look away from the target when starting the hit animation, and then swinging around to face them just before it lands so as not to trigger a kill move.
Normally this bug wouldnt bother me too much but having a build with 18 agility and max sneak I feel cheated everytime I get caught when I know I shouldnt.
