To actually answer your questions, how it work is that the game automatically save every single time you sleep in a bed. Even if you only sleep for a single hour. So if you die, whatever happen, your last save will wake you up in that bed like nothing happened. There is also a save function now which allow you to save about anywhere BUT it is only for convenience of being able to save if you need to stop playing right that moment and you don't have a bed nearby. The thing with that "save anywhere" is that if you end up dying later on you'll still reload at your last bed rest.
And yes, I would not play this game anymore without survival. I've found normal difficulty very lacking. I became a God very very fast, nothing was able to even hurt me much if at all anymore and I ended the main quest line quite fast. In Survival you actually have to monitor your health and hunger which is nice... But you also die very easily. It is also true of most enemies too, so it is way more realistic this way.
Actually surviving in survival mode is very satisfying, unlike in the vanilla normal mode game.

The other major draw back is no fast travel, makes things alot harder not being able to move quickly between area's, you''ll need to be a nuclear scientist just so your cores don't wear down so fast.
It's also bad if your game crashs or a settler you like die's in a raid and you just spent 2 hours in game and haven't saved, makes going back to do things a again always different because you'll probably end up doing something else.
