Though a close second would have to be Fog Crawlers, I mean listen to the sound it makes https://www.youtube.com/watch?v=3ZbWf2GGZTg Creeping around through the dense fog making those creepy sounds, you KNOW it's there but you can’t see it. then moments later is charges at you from out of the fog, it’s pretty awesome.

The ones that most often get me are radscorpions and mirelurks. Just minding my own business and all of the sudden I get one shot by a legendary glowing bug because my character got hung up on geometry.

OK, for me the "Creepiest" scare was when I first saw the Swamp people[family] in the Point Lookout DLC for FO3.

FUN FACT in the concept art the fog crawler was going to be the original design for the mirelurk queen but they scrapped it at the last minute, also centaurs were supposed to be in the game, the concept art for them looked disgusting.
