Make sure you have the unofficial fallout 4 patch, it helps lots, behind the scenes.
You also cannot have more than 128 interactable objects & NPC's (enemies, settlers, or companions/townsfolk) in one area at a time. The interactable, scripted object array tops at 128. That means, per settlement, per cell (chunk of game map, square in shape, like on the map in pip-boy), if it goes over, weird stuff happens!
I'll checkout the npc scaler and see whats up
any chance the level scaling mod works with Deadly Commonwealth Expansion?
