there are several things you can try to trouble shoot this if you don't have survival mode installed.
1. when health is declining, use a stimpack, should halt the damage effect.
2. try playing for 10 min in either god or immortal mode (console and type TGM or TIM)
do you have ANY mods installed? if you do it might be advisable to see if there are any conflicts between mods, LOOT will be able to tell you... BUT... if you do have conflicts it will be better to disable one of the conflicting mods then Start a new game as the script from the disabled mod will be imbeded in your old game causing other issues down the road. from uncompleteable quests to disapearing NPC's you never know what will happen. That's why when I intend to add new mods I make a hard save point so that if there are issues I can go back in time without too much loss.

Might target her in the console and try using the "moveto player" command on her.
I know when I have had followers stuck in the swim animantion on land fast traveling or changing places usually fixed the issue.
Not sure if they will work through console but there are these commands that might help
