About the missing ghouls, some are up in the roof or behind little wall holes. If you have sneaked through the whole tunnel, then it can be that the mark point did not got triggered (to "spawn" one or more ghoul(s) into the room through such holes). Also, there is one sleeping behind a locked cell door in the last room.
It can join the other bugged quests that sit in my log uncompletable (talk to jack Cabot about the artifact, Look at the Diamond City Bounty Board, etc).
Good thing is with it still "Active" no one else can have me chase down an enemy that is worlds away from them.
could be. It happens that a NPC/creature clips through the floor. Check the quest stage and proceed to the finished state if you want. The solution here below works if you are affiliated with the minute men because they issue you those "ghoul problem" quests. Therefore I think that you have that "Ghoul Problem" quest (check your quest list). If you have a different quest, please lookup the correct ID to modify the quest and its stages correctly.
This one gives you the quest stage
if it gives 100, then the quest is still to be executed (killing ghoulies)
this one moves the stage to the finished state, no ghoulies to kill anymore. You have to return to the settler/preston to claim your rewards, and with it, completing the quest.
You can find more of the story on a holotape near the train tracks police station. I think it is like right near the warehouse with the barred door and the molerats.
Fallout 76 was great with that same sort of story telling. Especially the the story/lore for the naughty nanny bot.
